About the Community Care Connection
In June 2011, Rosalind Franklin University Health Clinics took ownership of the CareCoach, a mobile health screening vehicle, from Northwestern Lake Forest Hospital. RFUHC gave the CareCoach a new look and a new name – the Community Care Connection. The vehicle is now back on the road, staffed by a dedicated team of medical professionals offering Lake County residents a variety of health screenings including blood pressure checks, blood sugar testing, cholesterol testing, body mass index (BMI) and waist measurements, and foot examinations – all free of charge. The Community Care Connection also provides interprofessional learning experiences for RFUMS students, who assist with the screenings under the supervision of licensed healthcare providers.
The Health Clinics has received generous support for the operation of the Community Care Connection in the form of grants from the Healthcare Foundation of Northern Lake County, Cardinal Health Foundation, and the Grant Healthcare Foundation. This support has enabled the Health Clinics to expand the number of sites and special events serviced by the Community Care Connection, as well as offering additional services such as sports physicals and osteoporosis screenings.
